Replacement Certificate Request Form 

Should a certificate be lost, stolen, or damaged, a replacement certificate may be requested. A replacement certificate may also be requested if your name has changed by submitting this form along with the Official Name Change Request Form.

Replacement certificates will be issued with the current certificate design and office holders' signatures (President and Dean). 

Confirm how you would like your name to appear on your certificate. If the name listed is different then the name on your Berklee Online account, please submit the Official Name Change Request Form.
Certificate Mailing Address

After all necessary documentation is submitted, your certificate will be mailed here. 


Please note: we do use a third party provider, Paradigm, for the printing and mailing of certificates. Due to this, certificates are only printed four times a year. Once your request has been submitted you will receive a separate email confirming the next round of printing.
